Poland sendromu ve spontan pnömotoraks nadir bir birliktelik

Abstract

Poland sendromu, tek taraflı pektoralis majör kası yokluğu, aynı taraf elde anomali ve birçok anomalinin birlikte olabileceği nadir bir konjenital anomalidir. İnsidans 1/7000 ile 1/100.000 arasında değişmektedir. Sendrom erkeklerde kadınlara göre daha sık görülmektedir. Yirmi iki yaşında erkek hasta acil polikliniğimize ani başlayan göğüs ağrısı ve nefes darlığı şikayetiyle başvurdu. Yapılan fizik muayenede solunum seslerinde sağ hemitoraksta azalma, sağ pektoralis majör kas yokluğu, sağ kolda kısalık, sağ omuz ve dirsek ekleminde hareket kısıtlılığı saptandı. Çekilen direkt akciğer grafisinde sağ hemitoraksta total pnömotoraks saptandı. Sağ tüp torakostomi ile tedavi edildi. Bu olgu Poland sendromu ve spontan pnömotoraksın nadir birlikteliğine dikkat çekmek amacıyla sunulmuştur.

Polands syndrome is an uncommon congenital anomaly characterized by unilateral absence of pectoralis major muscle, ipsilateral hand abnormalities, and a variety of associated anomalies. The incidence of the syndrome ranges from 1/7000 to 1/100.000. The syndrome is observed more frequently in man than in woman. A 22-years-old man who was admitted with a history of chest pain and dyspnea to our emergency department. On examination, breath sounds were decreased on the right side, there was an absence of the pectoralis major muscle, a shortness on the right upper limb and a restriction on the right shoulder and right elbow. Initially chest radiograph showed total pneumothorax on the right side. The patient was treated with tube thoracostomy. This case is presented to draw attention to the association of Poland syndrome with spontaneous pneumothorax.
