Amaç: Bilinen ve bilinmeyen birçok faktöre bağlı artmış reaktif oksijen türleri (ROS) spermatogenez ve sperm olgunlaşmasını etkileyerek erkek kısırlığına yol açmaktadır. Bu çalışmada idiyoptik subfertil olgularda egzersiz ve antioksidan gıda desteğinin seminal antioksidan kapasite, sperm DNA fragmantasyon indeksi, sperm kromatin niteliği ve sperm parametrelerine etkisi araştırılması amaçlanmıştır. Olgular ve Yöntem: Çalışma, Mart-Kasım 2021 tarihleri arasında Ondokuz Mayıs Üniversitesi Tıp Fakültesi’nde görülen idiyopatik oligoastenoteratozospermili subfertil erkekleri kapsadı. Tüm olgulara üç ay süreyle haftada 3-4 gün, en az 45 dakika (haftalık toplam en az 150 dk-600 MET) orta şiddette fiziksel aktivite önerildi. Toplam 48 olgu tam (basit) randomizasyon ile iki gruba ayrıldı. Birinci gruba (grup 1) günlük 2000 mg L-karnitin, 2000 mg fruktoz, 932 mg asetil L-karnitin, 225 mg vitamin C, 115 mg sitrik asit, 50 mg koenzim Q10, 14 mg çinko, 115 µg selenyum, 3750 µg vitamin B12, 500 µg folik asit içeren gıda desteği önerilirken, ikinci gruba (grup 2) antioksidan gıda desteği verilmedi. Tedavi öncesi ve sonrası semen analizi, hormonal değerlendirme, IPAQ anket formu ile fiziksel aktivite, Troloks eşdeğeri antioksidan kapasite (TEAC) ile seminal antioksidan kapasite, TUNEL yöntemi ile DNA fragmantasyon indeksi ve anilin mavisi boyaması ile sperm kromatin yapısı ölçüldü. İstatistiksel analiz Mann Whitney U ve Wilcoxon testler ile yapıldı Bulgular: Grup1 ve 2’ nin sırasıyla ortalama yaşları 33,54±3,6 ve 32,8±6,8, VKI 28,1±5,4 ve 28,3±3,5, 0. ay fiziksel aktivite değerleri 435,25±304,2 ve 498,12±353,4’dü. Değerler arasında anlamlı fark yoktu (p>0.05). Tedavi sonrası grupların TAC değerleri arasında istatistiksel fark yoktu (p=0,972). Antioksidan tedavi verilen olguların SDF indeksi (p=0,003) ve histon/protamin oranı (<0,001) önemli oranda azalırken, grup 2 deki azalma istatistiksel olarak önemli değildi. Sonuç : İdiyopatik OAT’lı olgularda antioksidan tedavi DFİ ve histon/protamin oranlarında anlamlı iyileşmeler sağlamaktadır. Aim: Approximately 30% of the factors that cause male infertility are due to idiopathic causes. Increased reactive oxygen species (ROS) due to many known and unknown factors cause male infertility by affecting spermatogenesis and sperm maturation. In this study, the effects of physical activity and antioxidant food supplementation on seminal antioxidant capacity, sperm DNA fragmentation index, sperm chromatin quality and sperm parameters were investigated in infertile cases. Material and Method: The study included subfertile men with idiopathic oligoasthenoteratozospermia seen at Ondokuz Mayıs University Faculty of Medicine between March 2021 and November 2021. All subjects were recommended to do moderate physical activity for at least 45 minutes (at least 150 minutes-600 METs per week) 3-4 days a week for three months. A total of 48 cases were divided into two groups by computer-assisted (www.randomizer.org) complete (simple) randomization. In the first group (Group 1), 2000 mg L-carnitine, 2000 mg fructose, 932 mg acetyl L-carnitine, 225 mg vitamin C, 115 mg citric acid, 50 mg coenzyme Q10, 14 mg zinc, 115 µg selenium, 3750 µg Food supplement containing vitamin B12 and 500 µg folic acid was recommended as one sachet in the morning and evening, while antioxidant food supplement was not given to the second group (group 2). Before and after treatment, semen parameters, Hormone analyzes with ELISA method, physical activity evaluation with IPAQ questionnaire, seminal antioxidant capacity with Trolox equivalent antioxidant capacity (TEAC) measurement method, DNA fragmentation index with TUNEL method. and sperm chromatin structure was evaluated by aniline blue staining. Student's t test was used for the variables showing normal distribution in independent groups, and Mann Whitney U test was used for the variables that did not fit the normal distribution. Paired t test was used for the variables showing normal distribution in the VI dependent groups, and Wilcoxon test was used for the variables that were found not to fit the normal distribution. Results: The mean age of the groups, BMI and physical activity values there was no difference (p>0.05). There was no statistical difference between the TAC values of the post-treatment groups (p=0.972). While the SDF index (p=0.003) and histone/protamine ratio (<0.001) were significantly decreased in the subjects given antioxidant treatment, the decrease in group 2 was not statistically significant. Conclusion: Antioxidant therapy provides significant improvements in DFI and histone/protamine ratios in patients with idiopathic OAT.
