Amaç: Günümüzde böbrek tümörlerinde, klinik T1 ve teknik olarak uygun T2 tümörler için kılavuzlarda yüksek kanıt düzeyi ve öneri derecesi ile önerilen ilk tedavi modalitesi nefron koruyucu cerrahidir. Parsiyel nefrektomi karmaşıklığını ve komplikasyon olasılığını tahmin edebilecek ilgili anatomik bulguları ölçmek için R.E.N.A.L. nefrometri skoru, PADUA skoru ve C indeksi gibi renal nefrometri skorlama sistemlerini kullanılır. Bu sistemler çoğunlukla kitlenin anatomik yerleşimi ile ilgili parametreleri göz önüne alarak değerlendirme yaparken hastaya ait özelliklerden olan yapışık perirenal yağ varlığını değerlendirmeye almazlar. Toksik yağ olarak da adlandırılan bu durum, inflamatuar yağlı dokunun böbrek ve tümöre yapışması ile böbreğin mobilizasyonunu ve tümörünün izolasyonunu zorlaştırarak parsiyel nefrektomide perioperatif morbiditeyi arttırması ile karakterizedir. Yapışık yağlı doku varlığının öngörülmesi amacıyla Davidiuk ve arkadaşları 2014 yılında "Mayo Yapışkanlık Olasılığı" skoru olarak adlandırılan kantitatif bir skorlama sistemi geliştirdiler. Operasyon sırasında toksik yağın en güçlü prediktörü olduğunu gösteren birçok çalışma ile bu skorlama sistemi ürolojide giderek günlük pratikte daha çok kullanılır hale gelmiştir. Çalışmamızda "Mayo Yapışkanlık Olasılığı" skoru ve preoperatif faktörler ile toksik yağ ilişkisinin incelenmesi ve intraoperatif ultrason kullanımının bu durumlarda operasyon aşamalarının süre ve komplikasyonlarına etkisi değerlendirilmiştir. Gereç ve Yöntem: Ekim 2010 ile Eylül 2020 tarihleri arasında laparoskopik parsiyel nefrektomi gerçekleştirilen 517 hastadan intraoperatif ameliyat görüntüleri ile preoperatif radyolojik imajları elde edilen, en az 1 yıllık takipleri yapılan 335 renal ünite çalışmaya dahil edildi. Bir radyoloji uzmanı tarafından tüm hastaların "Mayo Yapışkanlık Olasılığı" skoru perioperatif verilerden ayrı kör olarak hesaplandı. Olgular yağ özellikleri, "Mayo Yapışkanlık Olasılığı" skoru ve intraoperatif laparoskopik ultrasonografinin kullanımına göre ayrı ayrı değerlendirildi. Yağ dokusunun yapışıklığına bu konuda tecrübeli bir cerrah ile operasyon kayıtları izlenerek peritümöral yağ dokusunun makroskopik görüntüsü göz önüne alınarak "iyi" veya "kötü" şeklinde subjektif olarak karar verildi. Ayrıca operasyon aşamaları kayıtlar izlenerek, vasküler diseksiyon ile peritümöral yağlı doku diseksiyon süresi ve eksizyon ile sıcak iskemi süresi olarak not edildi. Bulgular: İyi ve kötü yağ sırasıyla 242 (%72,2) ve 93 (%27,7) olguda izlendi. Erkek (n=79 (%84,9), p<0,001) ve daha ileri yaşlı hastalarda kötü yağın daha sık olduğu izlendi (60,7 yıla karşı 55,1 yıl, p<0,001). Kötü yağa sahip olgularda kronik böbrek hastalığı evresi 3 ve 4 olanların daha fazla olduğu izlendi (%6,6'ya karşı %19,4, p<0,001). Kötü yağa sahip hastalardaki ekzofitik kitle oranının iyi yağ grubundan fazla olduğu görüldü (%80,6'ya karşı %61,2, p=0,003). Operatif sonuçlarda kötü yağ grubunda vasküler ve perirenal yağ diseksiyon süresi daha uzun izlenirken (p<0,001) kitle eksizyonu ve sıcak iskemi süresinin değişmediği görüldü (p<0,05). Perioperatif komplikasyon oranları arasında fark izlenmez iken trifekta ve pentafekta sonuçlarında istatistiksel fark izlenmedi (p>0,05). Multivariate regresyon analiz sonucunda erkek cinsiyet, preoperatif kronik böbrek hastalığı evresinin 3-4 olması, posterior perirenal yağ kalınlığı ve radyolojik yağ yoğunluğunun toksik yağı öngörmede güçlü parametreler olduğu bulundu (p<0,001). "Mayo Yapışkanlık Olasılığı" skoru için eğri altında kalan alan 0,857 olarak elde edilirken (p<0,001), 1,5 kestirim değeri için duyarlılık %80,7 ve özgüllük %74,1 olarak elde edilmiştir. "Mayo Yapışkanlık Olasılığı" skorunun operasyon aşamaları ile ilişkisi incelendiğinde, en fazla korelasyonun perirenal yağlı doku diseksiyonu ile olduğu tespit edildi (r=0,454, p<0,001). İyi yağ grubunda intraoperatif ultrason kullanımı ile perioperatif sonuçlar arasında fark izlenmezken, kötü yağ grubunda perirenal yağ diseksiyonu (p=0,014) ile kitle eksizyonu (p=0,050) ve toplam ameliyat süresinin (p=0,005) ultrason kullanılmayan grupta istatistiksel olarak daha uzun olduğu görüldü. "Mayo Yapışkanlık Olasılığı" skoru yüksek olan grupta da (4-5) benzer şekilde perirenal yağ diseksiyon süresinin ultrason kullanılmayan grupta daha uzun olduğu bulundu (15 dk'ya karşı 19 dk, p=0,029). Sonuç : Toksik yağ varlığı parsiyel nefrektomiyi zorlaştıran ciddi bir faktör olarak karşımıza çıkmakta ve yapışık yağın belli klinik ve radyolojik faktörler ile öngörülerek preoperatif planlama aşamasında intraoperatif ultrason gibi uygun cihazların temini ile radikal cerrahi için bir endikasyon olarak görülmesindense üstesinden gelinebilecek problem olarak görülmesi gerektiğini düşünüyoruz. Aim: Nowadays, nephron-sparing surgery is the first treatment modality recommended in the guidelines for clinical T1 and technically feasible T2 tumors, with a high level of evidence and degree of recommendation. Renal nephrometry scoring systems such as the RENAL, PADUA, and C index are used to assess the related anatomical findings that can predict the complexity and risk of complications of partial nephrectomy. The presence of adherent perirenal fat, which is one of the features of the patient, is often not measured by these systems, taking into account the parameters related to the anatomical location of the mass. This condition also referred to as toxic fat, is characterized by the adhesion of inflammatory fatty tissue to the kidney and the tumor, making it difficult for the kidney to mobilize and isolate the tumor, increasing perioperative morbidity during partial nephrectomy. Davidiuk and colleagues developed a quantitative scoring system in 2014 called the "Mayo Adhesive Probability" score to predict presence of adherent perinephric fat. In daily urology practice, this scoring system has become increasingly used, with many studies showing that toxic fat is the strongest predictor during operation. We assessed the relationship between "Mayo Adhesive Probability" score, preoperative factors, and toxic fat, and the effect of intraoperative ultrasound use on the duration and complications of the stages of operation in our study. Material and Method: The study included 335 renal units with intraoperative surgical records and preoperative radiological images collected from 517 patients who underwent partial laparoscopic nephrectomy from October 2010 to September 2020 and were followed for at least 1 year. The "Mayo Adhesive Probability" score of all patients was calculated by a radiologist, blinded from the perioperative data. The cases were analyzed separately on the basis of their fat properties, the "Mayo Adhesive Probability" score, and the use of laparoscopic intraoperative ultrasound. By considering the macroscopic appearance of the adipose tissue analyzing the operation records with a surgeon experienced in partial nephrectomy, adherent perinephric fat was subjectively defined as "good" or "bad". Moreover, the steps of the procedure were documented as the length of dissection of perirenal fatty tissue, vascular dissection, warm ischemia time, and excision. Results: Good and bad fat were observed in 242 (72.2%) and 93 (27.7%) cases, respectively. It was observed that bad fat was more common in male (n = 79 (84.9%), p <0.001) and older patients (60.7 years versus 55.1 years, p <0.001). Cases with bad fat were found to have more chronic stage 3 and 4 kidney disease (6.6 % versus 19.4 %, p <0.001). In patients with bad fat, the exophytic mass rate was found to be greater than that of the good fat group (80.6 % versus 61.2 %, p = 0.003). In the operative results, the duration of mass excision and warm ischemia did not alter (p<0.05) while vascular and perirenal fat dissection time was longer in the bad fat group (p<0.001). Although there was no difference in perioperative complication rates, the outcomes of trifecta and pentafecta were not statistically different (p>0.05). Male gender, preoperative chronic kidney disease stage 3-4, posterior perirenal fat thickness, and radiological fat density were found to be powerful parameters in predicting toxic fat (p<0.001) as a result of multivariate regression analysis. Even though the area under the curve was 0.857 (p <0.001) for the "Mayo Adhesion Probability" result, the sensitivity was 80.7 percent and the specificity was 74.1 percent for the cut-off value of 1.5. When the relationship between the "Mayo Adhesion Probability" score and the steps of operation was analyzed, it was found that perirenal fat tissue dissection was the most related (r = 0.454, p <0.001). Although there was no difference in the use of intraoperative ultrasound and perioperative results in the good fat group, perirenal fat dissection (p = 0.014), mass excision (p = 0.050) and overall operation time (p = 0.005) were statistically longer in the bad fat non-ultrasound group. Similarly, in the group with a high 'Mayo Adhesive Probability' score (4-5), perirenal fat dissection time was found to be longer in the group without ultrasound (15 min versus 19 min, p = 0.029). Conclusion: The presence of toxic fat emerges as a serious factor that makes it difficult for partial nephrectomy, and we believe that adherent fat should not be seen as an indication for radical surgery and should be seen as a challenge that can be handled by providing appropriate devices such as intraoperative ultrasound at the preoperative planning stage by predicting certain clinical and radiological factors.
