Publication: Veri Zarflama Analizi İle Performans Değerlendirmesi ve Sağlık Alanında Bir Uygulama
Abstract
Bu çalışmada Samsun 19 Mayıs Üniversitesi Eğitim ve Araştırma Hastanesine bağlı 27 anabilim dalının etkinliklerinin ölçülmesi amaçlanmıştır. Bu amaçla sağlık kurumlarının etkinliklerinin ölçümünde yaygın bir şekilde kullanılan ve parametrik olmayan etkinlik analiz yöntemlerinden olan Veri Zarflama Analizi (VZA) kullanılmıştır. Temel VZA modellerinden girdi ve çıktı yönelimli CCR (Charnes, Cooper, Rhodes) modeli ve BCC (Banker, Charnes ve Cooper) modeli ile analizleri gerçekleştirilmiştir. Bu bağlamda 2018 yılı içerisinde Ocak-Kasım ayları aralığında aktif ve yataklı poliklinik hizmeti veren 27 anabilim dalı incelemeye alınmıştır. Anabilim dallarından elde edilen verilerin etkinliklerinin hesaplanması ve diğer analizler için MaxDEA6 paket programı kullanılmıştır. Analizde dört girdi değişkeni ve üç çıktı değişkeni ile kullanılmıştır. Girdi değişkeni olarak her bir anabilim dalına ait Öğretim Üyesi ve Öğretim Görevlileri Sayısı, Asistan Doktor Sayısı, Hasta Bakıcı Sayısı ve Hemşire Sayıları dikkate alınmıştır. Çıktı değişkeni olarak ise her bir anabilim dalına ait Günlük Ayakta Muayene Sayısı (yıllık), Yatan Günlük Hasta Sayısı (yıllık) ve Ameliyat Sayısı kullanılmıştır. Elde edilen veri seti ile anabilim dallarının etkinlik skorları hesaplanarak, etkin ve etkin olmayan anabilim dalları belirlenmiştir. Etkin olmayan anabilim dallarının hangi etkin anabilim dallarını referans alarak etkin hale gelebileceği ve hangi güncellemeleri yaparak daha verimli bir anabilim dalı haline gelebileceği, arttırmaları veya azaltmaları gereken kaynaklar ortaya konulmuştur. Girdi odaklı ve çıktı odaklı CCR ve BCC analizleri incelendiğinde anabilim dallarının CCR modeline göre ortalama % 40 oranında etkin olduğu, BCC modeline göre ise ortalama % 46 oranında etkin olduğu gözlenmiştir
In this study, it is aimed to measure the activities of 27 departments of Samsun 19 Mayıs University Education and Research Hospital. For this purpose, Data Envelopment Analysis (DEA) which is one of the non-parametric efficiency analysis methods used in the measurement of the activities of health institutions, was used. Input and output oriented CCR (Charnes, Cooper, Rhodes) model and BCC (Banker, Charnes and Cooper) models of basic DEA models were analyzed. In this context, 27 departments providing active and inpatient services in January-November in 2018 were included in the study. MaxDEA6 software program was used for the calculation of the activities of the data obtained from the departments and other analyzes. In the analysis, four input variables and three output variables were used. As the input variable, the number of lecturers and instructors, number of assistant doctors, number of caregiver and nurses were taken into consideration. As the output variable, the number of Daily Outpatients (annual), Number of Inpatients (yearly) and Number of Operations were used for each department. The efficiency scores of the departments were calculated by using the data set and the effective and inactive departments were determined. How inefficient departments can become effective by referring to which effective departments and by which updates they can make to become more efficient department, the resources that are required to increase or decrease were revealed. When the input-oriented and output-oriented CCR and BCC analyzes were examined, it was observed that the departments were effective at 40% on average compared to the CCR model, and on average 46% on the BCC model.
