Bir köpekte domuz yaralaması sonucu oluşan uretral daralmanın perineal üretrostomi ile sağaltımı

Abstract

Bu çalışmada üç yaşlı, erkek, melez köpekte şekillenen travmatik striktür olgusunun perineal uretrostomi ile sağaltımı konu edildi Anamnezde, 3 ay önce bir domuz tarafından yaralandığı ve yaraların hasta sahibi tarafından dikildiği bildirildi. Dikiş bölgesinin iyileşmesinden sonra idrar yapmakta güçlük çektiği ve bunun giderek arttığı öğrenildi. Yapılan klinik muayenede postskrotal bölgede sirküler şekilde fıbröz doku palpe edildi. Sedasyon altında ince bir sonda ile idrar yolu kataterize edilerek direkt ve indirekt radyografileri çekildi ve daralmanın yeri tam olarak tespit edildi. Ultrasonografik muayenede idrar kesesinin dolgun ve çeperinin kalınlaştığı, böbreklerin normal olduğu saptandı. Kan analizinde üre nitrojen (BUN), kreatinin, total protein değerleri ile idrarda proteinin artmış olduğu belirlendi. Skatriks dokusunun perineal bölgede bulunmasından dolayı perineal uretrostomi yöntemi ile operasyona karar verildi. Operasyon sırasında ve sonrasında herhangi bir komplıkasyonla karşılaşılmadı. Ikı ay sonra yapılan kontrolünde uretrostomi bölgesinde herhangi bir daralma olmadığı ve idrarını rahatça yaptığı gözlendi Kan analizinde BUN, kreatinin ile total protein değerlerinin normal sınırlara yaklaştığı gözlendi. Çalışmamızda köpeklerde fazla tercih edilmemesine rağmen, skatrıksın bu bölgede bulunmasından dolayı perineal uretrostomi yöntemi uygulanmış ve herhangi bir komplıkasyonla karşılaşılmamıştır.

In this study, we discussed management of traumatic stricture in a three-year-old, male, mixed dog by perineal urethrostomy. It was informed that the dog was injured by a pig and its wounds were sutured by its owner. It was learned that dysuria occured after recover of sutured area and progressed this condition by the time. We determined that a circular fibrous tissue at postscrotal area during the clinical examination. Urethra was catheterized with a thinner catheter and then direct and indirect radiographs were taken to determine that the exact localization of stricture. Ultrasonographic examination revealed that the bladder was distended and thickened and, kidneys were normal. Blood BUN, creatinine and total protein were determined to be increased. Also proteinurie was detected. It was decided to perform perineal urethrostomy because of the scar tissue present at the perineal region. There is no complication observed during operation and postoperative period. The stricture at the new urethral opening was not excessive and did not prevent the dog from urinating easily after two months of operation. It was determined that the kidneys were normal and BUN, creatinine and total protein were nearly normal. In this study the perineal urethrostomy was applied because of the scar tissue present although this is not a preferable method in the dog and it was not observed any complications.
