dc.description.abstractBu çalışmada primer hiperparatirodi ile ilişkili nöral disfonksiyonun altta yatan patofizyolojisine katkı sağlamak amacıyla beyindeki belirlenmiş bölgelerin hacimsel ölçümleri elde edildi. Erişkin hiperparatiroidi tanısı konulmuş bireyler (19 kişi) hasta, sağlıklı ve nöropsikiyatrik şikayeti olmayan yetişkin bireyler (15 kişi) ise kontrol grubu olarak ele alındı. Katılımcıların restrospektif olarak alınan beyin manyetik rezonons görüntüleri Freesurfer programı kullanılarak analiz edildi. Belirlenmiş beyin bölgeleri için normalize veriler kullanılarak yapılan analizler sonucunda PHP li hastalarda sol hemisfer lingual hacminde hacimsel artış gözlenirken sol hemisferde insula, sağ hemisferde ise paracentral, pars triangularis, posterior cingulate, transverse temporal bölgelerinde azalma olduğu tespit edildi. Ayrıca normalize edilmemiş veriler kullanılarak hacimsel değişim üzerinde cinsiyet ve yaşın etkisi araştırıldığında, sağ hemisfer pars opercularis hacminin hasta grubunda azaldığı tespit edildi. Beyin bölgelerinin hacimsel değerlendirilmesinde azalmanın olduğu bölgeler ele alındığında, PHP li hastaların klinik bulgularında ifade edilen kemik ağrısı, yorgunluk ve nöropsikiyatrik bozukluklar ile bu bölgelerin işlevsel olarak ilişkili olduğu ifade edilebilmektedir. Bu çalışmada ikincil amaç doğrultusunda hacimsel olarak değişiklik gösteren beyin bölgeleri ile PHP hastalığında değişkenlik gösteren serum parametreleri olan fosfor, kalsiyum, parathormon ve vitamin D3 düzeyleri arasındaki ilişki lineer regresyon analizi ile incelendiğinde sol hemisfer lateral orbito frontal ve rostral anterior cingulate, sağ hemisfer superior temporal ve sağ hemisfer supramarginal hacimlerinin PHP li hastalarda, sağlıklı kontrollere göre ve tüm katılımcılarda yaşın artmasıyla azaldığı tespit edildi. Fosfor kalsiyum ve vitamin D3 düzeylerinin ise bölgelere göre değişen; hacimsel azalma, artma yada anlamlı etkiye sahip olma şeklinde sonuçlanan genelleştirilebilecek bir etkiye sahip olmadığı tespit edildi. Sonuç olarak bu çalışma PHP li hastalarda gözlenen (özellikle nöropsikiyatrik) şikayetlerin serum değerleri ile çok da ilişkili olmayıp beyindeki ilgili bölgelerdeki hacimsel azalma ile ilişkili olduğunu göstermektedir.en_US
dc.description.abstractThis study, volumetric measurements of designated regions of the brain were obtained to contribute to the underlying pathophysiology of neural dysfunction associated with primary hyperparathyroidism. Adult individuals diagnosed with hyperparathyroidism (19 people) were considered as the patient group, and healthy adults (15 people) without neuropsychiatric complaints were considered as the control group. Brain magnetic resonance images of the participants were analyzed with Freesurfer program. As a result of the analyzes using normalized data for the specified brain regions, it was determined that while there was a volumetric increase in the lingual volume of the left hemisphere in patients with PHP, there was a decrease in the insula in the left hemisphere and in the paracentral, pars triangularis, posterior cingulate, transverse temporal regions in the right hemisphere. In addition, when the effects of gender and age on the volumetric change were investigated using unnormalized data, it was found that the right hemisphere pars opercularis volume decreased in the patient group. Considering the regions where there is a decrease in the volumetric evaluation of the brain regions, it can be stated that these regions are functionally related to the bone pain, fatigue and neuropsychiatric disorders expressed in the clinical findings of patients with PHP. In this study, the relationship between brain regions that vary in volume and serum parameters that can change in PHP disease, like phosphorus, calcium, parathormone and vitamin D3 levels, was examined by linear regression analysis for the secondary purpose. It was determined that left hemisphere lateral orbito frontal and rostral anterior cingulate, right hemisphere superior temporal and right hemisphere supramarginal volumes decreased with increasing age in patients with PHP compared to healthy controls and in all participants. In case the levels of Phosphorus, calcium and vitamin D3, vary according to regions, it is obtained that these parameters do not have a generalizable effect that resulted in volumetric decrease, increase or significant effect. In conclusion, this study shows that the (especially neuropsychiatric) complaints observed in patients with PHP are not related to serum values but are related to volumetric reduction in the relevant regions of the brain.en_US
