Amaç: Bu çalışmada inferior alveolar kanal (İAK) ile gömülü alt üçüncü molar dişlerin ilişkisinin konik ışınlı bilgisayarlı tomografi ile (KIBT) incelenmesi amaçlandı. Materyal Metod: Çalışmamızda Ondokuz Mayıs Üniversitesi Diş Hekimliği Fakültesi Ağız Diş ve Çene Radyolojisi Anabilim Dalına çeşitli sebeplerden dolayı başvuran 20-70 yaş arasındaki hastalara ait, KIBT görüntüleri kullanıldı. Bu retrospektif çalışma kapsamında, İAK ve bu bölgedeki gömülü alt üçüncü molar diş bölgeleri net şekilde izlenebilen, 177 hastaya ait 260 (129 erkek, 131 kadın) gömülü alt üçüncü molar dişin KIBT görüntüleri incelendi. İlgili alanda kemik lezyonu olması, hastada travma öyküsü olması, gelişimsel anomaliler veya bölgesel patolojik değişiklikler hariç tutma kriterleriydi. Bulgular: Toplam 260 gömülü alt üçüncü molar dişin 120'si (%46,2) İAK ile ilişkili bulundu. 129 erkek hastaya ait KIBT görüntüsünün 57'sinde (%44,2) gömülü alt üçüncü molar diş ile İAK ilişkili iken, 72'sinin (%55,8 ) ilişkili olmadığı görüldü. 131 kadın hastaya ait KIBT görüntüsünün 63'ünde (%48,1) gömülü alt üçüncü molar dişler İAK ile ilişkili, 68'i (%51,9) ilişkili olmadığı görüldü. Winter sınıflamasına göre toplam 260 gömülü alt üçüncü molar dişin 105'i (%40,4) vertikal, 85'i (%32,7) meziyoangular, 54'ü (%20,8) horizontal ve 16'sı (%6,2) distoangular pozisyonda olduğu bulundu. Pell-Gregory genişlik sınıflamasına göre %56,2 oranında Sınıf II, derinlik sınıflamasına göre ise %55,4 oranında Seviye C olduğu bulundu. İAK ilişkili olan 120 gömülü alt üçüncü molar dişlerin İAK ile ilişkisine bakıldığında, İAK'nın 63 (%52,5) dişin köklerinin apikalinde, 38 (%31,6) dişin köklerinin lingualinde, 11 (%9,2) dişin köklerinin bukkalinde ve 8 (%6,7) dişin kökleri arasında bulunduğu belirlendi. İAK'nın ortalama kanal çapını bulmak için yapılan ölçümlerde kadınlarda ortalama kanal çapı 2,79 mm, erkeklerde ise ortalama kanal çapı 2,99 mm olarak tespit edildi. Sonuç: KIBT ile yapılan bu retrospektif araştırmada gömülü alt üçüncü molar dişlerin %46,2 oranında İAK ile ilişkili olduğu görüldü. Bu nedenle üçüncü molar dişlerin cerrahi işlemlerden önce üç boyutlu görüntüleme yöntemleriyle incelenmesinin gerektiği sonucuna varıldı.; Aim: In this study, it was aimed to examine the relationship between the inferior alveolar canal (IAC) and impacted lower third molars by cone beam computed tomography (CBCT). Materials and Methods: In our study, CBCT images of patients between the ages of 20 and 70 who applied to the Department of Oral and Maxillofacial Radiology of the Faculty of Dentistry of Ondokuz Mayıs University were used for various reasons. In this retrospective study, CBCT images of 260 (129 males, 131 females) impacted lower third molars of 177 patients, whose IAC and impacted lower third molar regions in this region can be clearly observed, were examined. Bone lesion in the relevant area, history of trauma, developmental anomalies or regional pathological changes were the exclusion criteria. Results: Of 260 impacted lower third molars, 120 (46.2%) were associated with IAC. While 57 (44.2%) of 129 male patients' CBCT images were associated with impacted lower third molars and IAC, 72 (55.8%) were unrelated. Impacted lower third molars were found to be associated with IAC in 63 (48.1%) of CBCT images of 131 female patients, and 68 (51.9%) unrelated. According to the Winter classification, 105 (40.4%) of the 260 impacted lower third molars were vertical, 85 (32.7%) mesioangular, 54 (20.8%) horizontal, and 16 (6%, 2) found to be in the distoangular position. It was found to be Class II at a rate of 56.2% according to the Pell-Gregory width classification and Level C at a rate of 55.4% according to the depth classification. Considering the relationship of 120 impacted lower third molars associated with IAC with IAC, 63 (52.5%) IAC were apical to the roots of the teeth, It was determined that the roots of 38 (31.6%) teeth were located lingually, the roots of 11 (9.2%) teeth were located buccally, and the roots of 8 (6.7%) teeth. In the measurements made to find the mean canal diameter of the IAC, the mean canal diameter was 2.79 mm in women and 2.99 mm in men. Conclusion: In this retrospective study performed with CBCT, impacted lower third molars were found to be related with IAC at a rate of 46.2%. For this reason, third molars should be examined with three-dimensional imaging methods before surgical procedures.

Paranazal sinüslerde yabancı cisim oluşumu oldukça nadir görülür ve sıklıkla maksillofasiyal travmayla ilişkilidir. Bu cisimler, yıllarca asemptomatik olarak kalabildikleri gibi maksiller sinüzit ve yabancı cisim reaksiyonu gibi bazı komplikasyonlara da yol açabilirler. Otuz üç yaşındaki erkek hasta, diş ağrısı şikâyetiyle kliniğimize başvurdu. Alınan anamnezde trafik kazası geçirme öyküsü olduğu öğrenilen hastanın, ağız içi ve ağız dışı muayenesinde ağrısı bulunan, sağ üst 2. molar dişinde çürük tespit edildi. Bununla birlikte, sağ yanak ve üst dudakta palpasyonda hassasiyet ve krepitasyon olduğu belirlendi. Alınan panoramik radyografide diş çürükleri ve kök artıklarının yanı sıra maksiller sinüs bölgesinde çok sayıda, farklı boyutlarda, heterojen yerleşimli, dörtgen şekilli, radyopak yabancı cisimler görüldü. Yapılan ultrasonografik değerlendirmede sağ yanak ve üst dudak bölgesinde yumuşak dokulara implante olmuş, çok sayıda hiperekoik yabancı cisim tespit edildi. Alınan konik ışınlı bilgisayarlı tomografi kesitlerinde sağ maksiller sinüsün anterior duvarını destrükte eden, maksiller sinüse yerleşmiş ve mukozal kalınlaşmaya neden olmuş, çevre yumuşak dokularda da izlenen çok sayıda, farklı boyutlarda heterojen yerleşimli, dörtgen şekilli, kemik dansitesinde yabancı cisimler izlendi.; Foreign body formation in paranasal sinuses is extremely rare and often associated with maxillofacial trauma. Foreign bodies may remain asymptomatically for years, however some complications such as foreign body reaction or pain may occur. A 33 year old male patient was admitted to our clinic with a complaint of toothache. In his anamnesis, it was learned that the patient had a history of a traffic accident. Intra-oral and extra-oral examination revealed a deep dentin caries in the right upper second molar, and tenderness and crepitation were detected on the palpation of the right cheek and upper lip region. In the panoramic radiograph, a large number of heterogeneously located, quadrangular, radiopaque foreign bodies in the maxillary sinus area were seen besides the decay teeth. Ultrasonographic evaluation revealed, a lot of hyperechoic foreign bodies implanted in the soft tissues of the right cheek and upper lip region. Cone beam computed tomography showed so many heterogeneously localized, quadrangular, bone density foreign bodies of different sizes in the antrum and in the surrounding soft tissues. In addition, mucosal thickening and destruction on the anterior wall of the right maxillary sinus were also observed.

PurposeThe aim of this study was to evaluate the efficacy of endoscopic visualisationto detect the presence and type of isthmuses within the mesial root canals ofmandibular first molar teeth compared with micro-computed tomography (micro-CT) images as reference.Materials and MethodsThirty-two mesial roots of mandibular first molars presenting isthmuses wereselected based on micro-CT scans. In all, 12 type I and 20 band-shaped isthmuseswere collected. The specimens were mounted in the posterior socket of dentalphantom manikin for endoscopic visualisation. The ability of endoscopes to visualizethe presence of isthmuses and distinguish the type of isthmuses was compared.Micro-CT images of the specimens were used as references. Data were analyzedusing Fisher’s exact tests.ResultsSensitivity of endoscope to detect isthmuses were also calculated for eachisthmus type. In 37.5% of the samples, isthmus presence was correctly diagnosedvia orthograde endoscopic visualization. Type I istmuses were significantly moredetected than band-shaped isthmuses (P<0.05). Endoscope showed highersensitivity to detect type I isthmus than band-shaped isthmus.ConclusionThe endodontic endoscope could detect type I isthmuses more accurately thanband- shaped isthmuses.
